HOW TO FILE A CLAIM

Submission by email or post:

  1. Complete the Claim Form and gather supporting documents
  2. Submit the documents (completed claim form and all supporting documents)
  3. Claims Assessment by Generali    
  4. Settlement email notification will be issued to you after claim is assessed and proceeded 


SUPPORTING DOCUMENT(S) CHECKLIST

OUT-PATIENT CLAIM

  • Claim Form with signed and completed in full.
  • Attach all original payment bill(s), receipt(s) and claims supporting documents/reports.
  • All bill(s) and receipt(s) must clearly show the date of treatment, patient’s name, diagnosis, breakdown of charges and the attending physician’s stamp and signature.

 

HOSPITALIZATION & SURGICAL CLAIM

  • The policyholder or the patient should complete Part I of the Claim Form, while Part II should be completed by the attending physician.
  • All original bills and receipts must be attached showing the date of treatment, patient’s name, diagnosis, breakdown of charges and the attending physician’s stamp and signature.
  • If the hospitalization and/or out-patient surgery was made outside Hong Kong Special Administrative Region, please provide claim supporting documents in English or Chinese.


IMPORTANT NOTE

OUT-PATIENT CLAIM:

 

  • Claim must be submitted and received by the Generali Service Center within 90 days from the date of treatment.
  • Please ensure that all fields on this Claim Form are completed and that all required claim documents are submitted. Incomplete forms or omissions may cause processing delays. Additional information related to this claim may be requested.
  • If the submitted claim is approved, the claim payment (in Hong Kong Dollars) will be reimbursed by autopay to the bank account most recently specified by the policyholder and recorded with Generali.
  • If the hospitalization was made in a Hong Kong Special Administrative Region Hospital Authority’s Hospital, please attach the Discharge Summary for provision of diagnosis and surgery information.

 

HOSPITALIZATION & SURGICAL CLAIM:

 

  • Claim form must be sent to the Generali Assistance Centre within 90 days from the date of discharge/treatment.
  • Please ensure that all fields in Part I and Part II of this Claim Form are completed and that all required claim documents are submitted. Incomplete forms or omissions may cause processing delays. Additional information related to this claim may be requested.
  • If the submitted claim is approved, the claim payment (in Hong Kong Dollars) will be reimbursed by autopay to the bank account most recently specified by the policyholder and recorded with Generali.
