- Claimant needs to bear the fee of completing claim form part II
- We may ask for other documents or supporting information on case by case basis to facilitate the claim process.

Step 1: Complete the Claim Form and gather supporting documents
Step 2: Submit the documents (completed claim form and all supporting documents)
Step 3: Claims Assessment by Generali
Step 4: Settlement letter and/or email will be issued to you after approval

- Sick Leave Certificate
- Patient Card Copy
- Medical and / or Laboratory Test Report
