HOW TO SUBMIT A CLAIM

For submission by email or post:

Step 1: Complete the Claim Form (Full version/For Hospitalisation and Surgical Expenses/Outpatient Expenses) and Gather & submit the claims supporting documents 

Step 2: Claims Assessment by Generali     

Step 3: Settlement letter and/or email will be issued to you after approval

SUPPORTING DOCUMENT(S) CHECKLIST

Please keep the original receipt(s) and other claim documentation for 6 months from the claim completion date. Generali reserves the right to request additional information/documentation to support each claim.

1. For Hospital and Surgical Claim
  • The original hospital/medical bill(s)/receipt(s) certified by the attending doctor and stating clearly the diagnosis.
  • Written referral letter by registered doctor for laboratory tests/x-ray/specialist consultation (if applicable)
2. Outpatient/Dental Claim
  • The original medical receipt(s) F18 attending doctor/dentist clearly showing the date of treatment, patient's name and diagnosis/type of dental treatment; the policy number and the name of the Insured must be clearly written on the back of the receipt(s)
  • Written referral letter by registered doctor for laboratory tests/x-ray (if applicable)

IMPORTANT NOTES
  • Please state clearly your policy number and name of insurer in the communication.
  • If you have other documents which are not listed but may support your claim application, please also submit them as it may facilitate our processing.
  • If there is another insurance policy covering the same loss or damage, you should provide a copy of the full set of the policy to us.
