HOW TO FILE A CLAIM

Submission by post 

Step 1: Complete the Claim Form and gather supporting documents  

Step 2: Submit the documents (completed claim form and all supporting documents)

Step 3: Claims Assessment by Generali     

Step 4: Settlement letter and/or email will be issued to you after approval

SUPPORTING DOCUMENT(S) CHECKLIST

  • Proof of Identity of the Insured and Policyholder (if not provided before) 
  • Sick Leave Certificate
  • Patient Card Copy
  • Medical and / or Laboratory Test Report

IMPORTANT NOTES
  • For any documents not written in English / Chinese, the Policyholder needs to provide the certified valid translation 
  • Claimant needs to bear the fee of completing claim form part II 
  • We may ask for other documents or supporting information on case by case basis to facilitate the claim process.
CLAIMS SUBMISSION

Please submit written notice and written proof of claims within a specific period of time (please refer to the policy provisions on the required timeframe).
